2020 Fond du Lac Yacht Club Slip Request Form

The 2020 Boating Season will be upon us before too long and we will have at least one slip available for lease next year.  If you are interested in putting your name on the list to be considered for the slip, please fill out the form below by following the link and providing the information requested.  This information will be stored and sent to the Dock Committee Chairman so it can be used to assign the slip in the coming months.  This process must be completed every year.

To have your name added to the list simply follow these instructions.  It is recommended that you read through the instructions first before heading to the form to complete it.  Here we go:

1)  Gather the information about your boat such as draft, length, and width.

2) Click this link to be taken to the form:  2020 Fond du Lac Yacht Club Slip Request

3)  You will be asked to sign in as a member.  YOU DO NOT NEED TO SIGN IN AS A MEMBER. However, you will be required to add some additional information that will transfer over to the form.  (Benefit of signing in as a member is that you do not have to enter anything other than your boat information).

4)  Complete all fields of the form.

5)  Attach proof of ownership.  This file can be uploaded as an Adobe PDF or a photo file.  The choice is yours.

6)  Click SUBMIT.  Your information will be sent to the Dock Committee Chairman for use in the 2020 Slip Assignment process.
